Participant(s) & Form Signers Agree:

Being an adult, (or a minor on whose behalf a parent or legal guardian is signing this document) certify that I (or the minor participant) can swim and acknowledge inherent risks of swimming and board sports in the ocean and potential injuries that may result by participating in water board sports. I agree that surfing, skimboarding, and skateboarding are participation sports and the use of these boards involves a risk of injury to any and all parts of my body. I hereby agree to freely and expressly assume and accept any and all risks of injury while participating or spectating. I agree that I release STEFF MAGS LLC, SANDY AVOCADO SURF, the town in which the lessons are performed, & all of its successors, employees, affiliates, officers, directors, and agents from any and all responsibility of liability for injuries or damages of any nature occurring from participation in SANDY AVOCADO SURF activities regardless of whether it was caused or contributed to by the negligent conduct. I also expressly agree to release and discharge STEFF MAGS LLC from any act or omission in rendering or failing to render any type of rescue emergency or medical services. I agree to not make a claim against or sue STEFF MAGS LLC or the town in which the lessons are performed for injuries or damages related to SANDY AVOCADO SURF. I agree to release STEFF MAGS LLC and the town in which the lessons are performed from any such responsibility which results from the use of surfboards, skimboards, and skateboards. I hereby agree to release STEFF MAGS LLC and the town in which the lessons are performed from any and all liability. I understand that SANDY AVOCADO SURF provides the safest environment possible for participants and spectators and further understand that STEFF MAGS LLC is not obligated to provide safety equipment to participants. I consent to the use of a beach tent during operational hours and release STEFF MAGS LLC, the city and town in which the lessons are performed, any inherent risks associated with the use of this object. I also authorize SANDY AVOCADO SURF/STEFF MAGS LLC to use any photos or video taken of myself or the minor whom I am signing for social media and advertisements. I have read and understand the agreement. I understand that by making and signing this agreement, I surrender valuable rights, including but not limited to sue Steff Mags LLC, and all of its successors, assigns, affiliates, officers, directors, employees, and agents for any common law negligence, or liability.
